Jumping in here some time later as I re-read this thread and want to point you to a tool that might be helpful in some situations, which is a Word-to-epub plugin for MS Word. It was designed as an accessibility tool and I use it regularly in my job so I can vouch that it is very easy to use, is fairly customizable for output, and produces a really nice, clean epub.
